(First Adult Passport / Replacement of Emergency Passport / Replacement of Lost, Stolen or Seriously Damaged Passport / Previous passport expired for more than 3 years)

Guarantor

(Australian citizen or foreign citizen currently employed in an  approved occupation)

(1 photo must be appropriately endorsed and signed by the Guarantor)

(Current or expired passport, including a previous child passport or emergency document)

Replacement Indian birth certificates can be obtained from the Municipal Corporation / Registrar of Birth Deaths and Marriages office where the birth was originally registered

 

English Translation

(only required if document is not issued in English. Originals only)

**Dual language certificates (e.g., Indian Language and English) are acceptable

 

Proof of Australian Citizenship (Australian Citizenship Certificate)

(First Child Passport / Child Passport Renewal/ Replacement of Emergency Passport / Replacement of Lost, Stolen or Seriously Damaged Passport)

Parental consent page completed by all persons with parental responsibility

(Signatures must be witnessed appropriately, Page 4 of application)

(Current or expired passport, including an emergency document)

 

Children aged 10 and over must sign the application form (section 18)

 

Lodging parent/s must provide Photo ID with address and signature at lodgement

Child applications aged 15 or under must be lodged by a parent or person with parental responsibility for the child

 

Child applications aged 16 or 17 must be lodged by the child applicant together with a parent or person with parental responsibility for the child.
